What's the difference between this one and the one prior?
I'm so confused. Is it new version?
But what's the difference between these two 7458 versions? This one has (Std) in title, the other one has "w/ USB". But the statement in this one does say USB support is included. The size of this firmware is 3M, while the other one is 6M.
I understand your confusion now. I copied the description from the previous download, which contains features that do not pertain to this non-USB version.
Toastman's download site doesn't seem to have changelogs, so I can't confirm exactly what changed from version to version.
However, the firmware itself (which I installed) does have info in the about page, which I'll post here, and will update in the download itself.

I also just noticed that these notes do indicate the standard (std) version supports USB, so I'm a bit confused, too. I haven't tested USB yet.

The build nomenclature is exactly that used by Teddy Bear, the author of TomatoUSBmod and Tomato-RT upon which these builds are based. Here is the info page:

The latest build 7462 allows you to configure your own QOS class names. In future there will therfore be no need for me to compile two versions of the release, one with Toastman class names, and one with the original names. Toastman class names will be installed by default if you erase NVRAM, and then you can change them to anything you like.
