I use dd-wrt 11218 miniBuild (2,8MB) on my 614Lv8 WW Model.

This build has no openvpn Services integrated.

Only the unsecure PPTP

Q1: 

I have read, 614Lv8 WW not the NA Model  can't install *.bin files with the GUI from dd-wrt.

WW 614Lv8 Model can only use *.chk files, per tftp to upgrade

Is that right?

There is no Std/smal 11218.chk in Elko's new builds, only Std/Smal *.bin for 614Lv8 .

Q2:

rename Std/smal *.bin in *.chk is that a way?

Yes, only *.chk files are upgraded into the board through GUI or tftp.
If you have one bin file then you have to convert it to chk file. Only renaming the extension will not solve this problem. You can try this tool to convert it to chk image. http://www.myopenrouter.com/download/10611/mkchkimg/

It is not clear from your post why do you need the CD .... the reset button on the board can be used to get back factory defaults .

Then you can access board's GUI at 192.168.1.1 and configure the board as per your need

What do you want to do with Netgear router ? It is not clear to me why you need CD to access the router.
