I have Tomato v1.11 on my WGR614v8 router. I have a client interested in using access control to limit some workstations to around 50 approved websites and it looks like the access rules will work better than in Tomato which appears you can only add sites to deny access to. Should I reset the router back to the default Netgear firmware, then to DD-WRT to be safe?

you can get back to the Netgear firmware or ccan install ddwrt but I guess all of these firmware allows to configure the sites which you want to block. They do not allow you to configure url that you want to make accessible.
